You can submit a Service Request by clicking "Members Only" on the menu bar and selecting "Submit a Request." This brings up the page shown in Figure 1. This page contains your personal contact information but is for reference only and cannot be changed on this form. It's to let you know that we know it is you making the request and to let you check that the information about you that will be provided with the request is correct. (If you need to make changes to this information, you can do so from the "Contact Information" menu item of the 'Members Only" menu.)
To make a new request to management: 1) Select the subject from the "Subject" drop list, 2) select the priority, 3) enter your request and then, 4) click the "Submit" button. A confirmation message will pop-up confirming that the message was sent; and after your manager reads your request an automatic email will be generated letting you know that it has been read.
